Home and Live-In Care Services. Care for Me can arrange a meeting with you or your loved one for assessment with one of our Nurse Managers to determine a Comprehensive Care Plan that would be personalised to you or your loved one needs. This meeting may also include your GP or Consultant and can be carried out at home or in hospital prior to discharge. If this is the case the Comprehensive Care Plan may be drawn in conjunction with a medical care plan from your GP or Consultant.
We will give you all the information you require about Care for Me and our services i.e. Nursing Care at home or non-medical with Care Assistants. Before you meet with us it is a good idea to write down all of the questions you may have at this stage.
If you decide after our initial meeting that you require the services of Care for Me we will be more than happy to start the care package you have chosen on a day / date that is suitable. The Care for Me Manager will always introduce the Certified Nurses or Care Assistants to you and your family / loved one and go through the comprehensive care plan again.
We understand care plans can change very quickly particularly with complex medical issues. You can rest assure that our Certified Nurses or Care Assistants can perform a full array of live-in care services, so your loved one can remain in the safety and familiarity of their own home, no matter what their condition.
Care for Me provide a wide range of home nursing from oncology to cardiology, from surgical to medical, dementia and care of the elderly to palliative care. We also have a team of competent and experienced carers for patient support, when nursing care is not required.
Care for Me recognizes that Alzheimer’s and dementia affects each person differently, meaning there is no one way to provide care. That is why we will work with you and your loved one to determine what services are most important for their safety, mental health and physical well-being.
Live-In Care services can include:
- Carry out Physician’s Medical Plan of Treatment
- Regularly Report to Client’s GP or Consultant
- Monitor Complex Health Conditions
- Assistance with Medical Equipment
- Catheters
- Tube Feeding
- Dressing Changes
- Wound Care
- Teaching about care (injections, suctioning, etc.)
- Medication Management & Assistance
- Trach Care
- Assistance getting in and out of bed (Transfers) including hoist
- Toileting, Bathing & Personal Hygiene
Companion Care Services
Companion Care is an important part of the in-home care process, allowing people who don’t require extensive care and assistance the ability to remain at home without being lonely or unsafe. The Companion or Homemaker care providers that we refer perform several services for those who don’t require “hands on” personal care, but need a little help to remain at home. All of these services can be personalised to meet your needs and of your family.
Respite Care Services
Respite care is all about taking care of the family caregiver. We understand that caring for an elderly or ill loved one can take a toll on your personal and professional life. Care for Me can meet with you and your loved one for assessment. During the assessment, we can create a Care Plan that accommodates the best care possible during the respite period.
